Why do I receive multiple bills from LMHS?
There are different areas in the health system which send
bills for their individual services. For example, the emergency
room, outpatient services, inpatient services, physicians, wound
care and home health all send their own bills. A patient who
stays in the hospital will receive a statement from each
physician involved in his or her care and a statement from the
facility where he or she stayed.

Does my physician accept
my insurance?
Lee Physician Group and Lee Physician Specialists currently
accept Aetna, BCBS, Cigna, Humana, Medicare, Medicaid, United
Healthcare, Amerigroup for OB/GYN, Staywell Healthy Kids and
Staywell Medicaid. They also accept Lee PHO.

Where should I call to
update and/or re-bill my insurance?
If you are a Lee Physician Group patient, call 239-424-1400.
If you are a Lee Physician Specialists patient, call
239-424-1499. Please have your insurance card available when you
call.

How do I get a copy of
an itemized bill?
For a copy of an itemized bill from Lee Physician Group,
call 239-424-1400. For a copy of an itemized bill from Lee
Physician Specialists, call 239-424-1499. You will need to tell
the representative your name, date of birth, account number and
date of service. You will receive the itemized bill at the
current address listed on your account within 10 days.

How can I check the
status of my account?
If it has been at least 45 days since the date of discharge,
contact your insurance carrier for the status on the claim. If
you still have concerns regarding the claim after speaking with
your insurance carrier, contact Lee Professional Billing at
239-424-1400 if the bill is from Lee Physician Group, or
239-424-1449 if the bill is from Lee Physician Specialists.

Where can I obtain a
good faith estimate of fees for treatment?
A good faith estimate of reasonably anticipated fees is
available from your physician's office. The estimate will be the
average charge for the requested procedure. The actual charge
may exceed the estimate. Please contact your physician's office
for additional information related to fees and payments.

Do the physician offices
accept letters of protection from attorneys?
If it has been at least 45 days since the date of discharge

How will I know if my
physician bill has been paid?
Your insurance carrier should send you an explanation of
benefits, or EOB, indicating that the claim has been processed.
The EOB will also show any payments made to your physician and
the amount that is your responsibility. You will receive a
statement from Lee Professional Billing once your insurance been
processed. This statement will reflect all insurance payments,
adjustments and any balance remaining. The remaining balance is
your responsibility.

Lee Physician Group patient or 239-424-1449 if you are a Lee
Physician Specialists patient.

Can I make a payment
with a credit card?
Yes, we accept all major credit cards - Visa, MasterCard,
American Express and Discover - including debit cards.

Yes, leave a message with your name, account number and
phone number. We will call you the next business day.

What information do I
need to pay my physician bill?
To make a payment, you will need to have your billing
statement available. The statement will provide you with the
required patient account number to make an online payment. You
will also need to know the patient's date of birth.

your account number is located on the statement.
