Insurance and Sample Statements
As a convenience to you, we
file your hospital claim your insurance carrier based on the
information provided at the time of registration. The Central
Business Office staff will follow-up with your insurance company
for payment. If the insurance payment is not received within
forty-five (45) days you will receive a letter from the Central
Business Office requesting that you contact your insurance
carrier directly to ensure that they have received your bill and
are processing payment on your behalf.
After all insurance
payments have been applied to your account; you will receive
statements from Lee Memorial Health System showing the amount
paid by insurance and the amount due from you. Please ensure
that the balance is paid in full by the date on the statement.
In addition to the Lee Memorial Health System statement, you
should receive an Explanation of Benefits from your insurance
company.
If you do not have
insurance, you will receive a statement indicating your total
charges and balance due. Payment of the full balance is expected
by the date on the statement.
