your first visit, please have the following information

telephone of treating physician

Insurance information

If you have an HMO
insurance plan, contact your primary care physician to
obtain authorization for a specialist visit. It is the
patient's responsibility to ensure authorization is obtained
prior to arriving for the appointment.
Please bring all medical
records related to your diagnosis with you to your first visit,
including notes, slides and films from scans.
