The staff of Lee Diabetes
Care is dedicated to providing the most comprehensive
educational program available. Working with your physician, our
team of specialists will help you design an individualized
program to control your diabetes and have a healthier lifestyle.
Lee Diabetes Care offers
the Diabetes Self-Management Education Program at four locations:
Cape
Coral Hospital, HealthPark Medical Center,
Lee Memorial
Hospital, and Gulf Coast
Medical Center. You must be referred to us
by a physician to participate.
Medicare (certain
criteria applies) will cover 80% of the cost of the program; we
can bill your supplemental insurance for the balance.
Additionally, most HMO, PPO and commercial insurances will cover
the cost as well.
Family members are
encouraged to take part in each of the programs to assist you in
keeping your life in balance.
