Levels of Giving

Donors to the Lee Memorial Health System (LMHS) have helped raise millions of dollars to fund lifesaving care for children and adults in Southwest Florida. Your support helps fund critical research, advanced technology, skilled and compassionate care and helps support our mission to be the best Patient-Centered health system in Florida. Lee Memorial Health System Foundation is truly grateful to all of its supporters.

Visionaries $1,000,000 +

Visionaries will receive all of the below benefits plus: Complimentary admission to all Foundation events (2008-2009): Minnesota Twins- Dinner with the Twins and Celebrity Golf Classic; Boston Red Sox Diamond Dinner, Tee Party, The Children’s Hospital Celebrity Classic; and Saks 5th Avenue Key to the Cure Exclusive invitation to dinner with Jim Nathan, LMHS President/CEO

Humanitarians $500,000- $999,999

Humanitarians will receive all of the below benefits plus: Complimentary table at one Foundation event

Benefactors $250,000- $499,999

Benefactors will receive all of the below benefits plus: Exclusive invitations to financial planning updates Preferential seating at Foundation events

Founders $100,000- $249,999

Founders will receive all of the below benefits plus: Private behind-the-scenes hospital tour Subscription to LMHS Healthy News Exclusive invitation to breakfast with member(s) of LMHS Leadership Opportunity to meet Jim Nathan, LMHS President Opportunity to be profiled as a “Donor Next Door” in the Foundation's quarterly newsletter Opportunity to have press release sent out announcing your generous support of LMHS Naming opportunity with private reception.

Patrons $50,000- $99,999

Patrons will receive all of the below benefits plus: Exclusive invitation to lunch and medical briefing on relevant health topic, e.g., cancer prevention, heart health, pool safety or other offering Recognition on Foundation website for corporate sponsors.

Ambassadors $25,000- $49,999

Ambassadors will receive all of the below benefits plus: Advanced notice of system events Special listing in Evening of Appreciation program

Supporters $10,000- $24,999

Supporters will receive all of the below benefits plus: Permanent recognition on a plaque of increasing prominence on LMHS Donor Wall* at all five medical centers

Friends $1,000 – $9,999

Friends will receive all of the below benefits plus: Recognition on the LMHS Donor Wall at all five medical centers for the period of one year Exclusive invitation to the Evening of Appreciation, donor recognition dinner Invitation to annual Evening of Appreciation donor reception

Associates Up to $999

Associates will receive: Mailings of the Foundation's quarterly newsletter, and will be recognized in the LMHS Foundation Annual Report
