Solutions, a Lee Memorial Health System Department created to
help patients manage chronic health conditions. Lee Health
Solutions has programs for diabetes, weight management.
Lee Health Solutions began
with just one program, Lee Diabetes Care. Our success in helping
those with diabetes to live better, fuller, healthier lives
convinced us to expand our services to help those with other
chronic conditions that diminish the quality of life.
The prevalence and cost of
chronic disease in the United States is staggering:

Americans suffer from at least one chronic condition.

result in 70% of all deaths in the nation.

The medical costs for
patients with chronic disease makes up 75% of the nation's
$2 trillion in healthcare costs.

Nearly 23.6 million
Americans have diabetes, and 224,092 people die from it each

Americans have undiagnosed diabetes.

It is estimated that 66% of American adults are classified
as overweight and obese.

Americans suffer from congestive hearth failure, with
550,000 new cases diagnosed annually.
