We provide in-home elderly
care delivered by highly skilled and compassionate health care
professionals is in great demand. As the population continues to
age, more families will be faced with the decision of at home
care versus care in an assisted living or skilled nursing
facility.
For many elderly, the
desire to live in their own home setting can be quite
challenging and often times impossible, when they have no one
nearby to turn to for assistance.
Senior Care Choices
provides assistance to seniors who want to live independently,
and families concerned about their loved one's daily care. Our
objective is to help provide a high level of personal attention
and reassurance for your family member at a reasonable cost.
Our involvement begins with
a referral from you, or the client, a physician or other health
care worker, a friend or community agency. One of our case
managers will arrange to do an in-home assessment based on your
concerns. This assessment addresses the health, psychosocial,
financial and safety needs of the client in order to determine
what type of assistance is needed.
