Comprehensive Cardiac Care
Lee Memorial Home Health specializes in
comprehensive cardiac care offered by experienced cardiac
clinicians. The cardiac clinician's objective is to prepare the
patient to manage his/her disease processes in their home
environment, preventing re-hospitalization. Care for the cardiac
patient includes complete assessment with interventions,
evaluations, effectiveness of interventions, patient education
in the management of cardiovascular disease, and medications.
Services provided may include point of care
testing for coumadin therapy, pulse oximetry, physical therapy
to assess the effectiveness of exercises and other therapeutic
regimes, assisting in behavior modifications, and having home
health aides assist patients with daily living activities to
foster independence.

Infusion Therapy
Lee Memorial Home Health provides patients
with Enteral/Infusion Services. Our team and Access Infusion
Partners have a joint venture, which has been in place for 13
years. We provide the nursing services, and Access Infusion
Partners provides the IV pharmacy needs. Those needs include
enterals, IV antibiotics, nutritional supplements, pain
management, central line and tube feedings, other IV
medications, and all the supplies associated with IVs (alcohol
swabs, globes, IV dressing change kits, syringes and
pharmacological support).

Ostomy and Wound Care Management
Lee Memorial Home Health Ostomy and Wound
Care Management Services team is comprised of nurses who
specialize in the care of chronic wounds and ostomies. These
nurses provide teaching in all of the LMHS hospitals and offer
further training in the home after the patients are discharged.
The goal of these services is to promote healing and self-care,
using an individual approach to the patient's needs.

Diabetes Management
Lee Memorial Home Health provides
comprehensive diabetes care for patients of all ages. Our
nursing staff works with the patient's physician to develop and
individualized plan of treatment to help meet your specific
goals. Our nursing staff will teach you about your medications,
monitor your blood sugar levels, teach you about diet, look for
signs and symptoms of hyper- and hypoglycemia, and teach you
about the disease process. In conjunction with your physician,
our physical therapists will evaluate your need and develop a
home exercise program designed to help you maintain a healthier,
more active lifestyle that will reduce complications of
diabetes.

Pediatric Services
Lee Memorial's Home Health Pediatric
Program is dedicated to serving and supporting our Children's
Hospital and the families of Lee County. Our experienced staff
is committed to uncompromising excellence by providing quality
home health care as part of Lee Memorial's commitment to
superior service that extends even beyond the hospital stay. Our
Pediatric Program specializes in caring for infants, children
and adolescents. These services include newborn assessment,
phototherapy, wound care, IV therapy, diabetic teaching,
parenteral and enteral nutrition, oncology/hematology, lab work,
nebulizer treatments, asthma education, apnea monitoring,
medication teaching, and gastrostomy tubes. We assist with
coordination for a smooth transition from our hospital to your
home, providing family education and support services for
chronically ill children.

Perinatal Services
Meeting the special healthcare needs of
pregnant women with skill and compassion is the goal of Lee
Memorial Home Health. Our perinatal outreach program works
closely with the physician to provide comprehensive,
high-quality care in the comfort and familiar surroundings of
home. Pregnancy can place a variety of stresses on the body, and
your physician may recommend home monitoring in cases of
pregnancy induced hypertension (high blood pressure), diabetes,
wound infections, pre-term labor, pregnancy induced nausea or
vomiting. Our services include non-stress testing, heparin
therapy (blood thinning), intravenous hydration and antibiotic
therapy, post-partum care, breast feeding education, lab work,
and gestational diabetes education.

Pre-Fill Medication Management
Lee Memorial Home Health offers a pre-fill
medication service to assist patients on either a weekly or
bi-monthly basis. A registered nurse will review current
medications, dosages, and possible complications, then consult
with the patient's physician regarding new drugs or changes to
existing medication. Each visit includes a comprehensive
assessment (blood pressure, temperature, heart rate, and more)
and discussion with the patient's pharmacy for medication
refills. Medication issues, such as usage, side effects and
contraindications, are explained during each visit. Medicare or
private insurance does NOT cover this service. The goal of our
program is to foster independence and assist in maintaining
health with medication management.

Rehab Services
Lee Memorial Home Health Rehabilitation
plays a vital role after recovery of major orthopedic,
neurological, cardio-pulmonary and general surgeries. Rehab is
also important when recovering from major neurological events
like strokes. Licensed Physical Therapists, Speech Pathologists,
and Occupational Therapists treat patients. The home
rehabilitation process is usually initiated by a physician
referral. After an evaluation, the licensed therapist designs a
unique treatment plan based on the patient's needs. The patient
and caregiver are closely involved in establishing the treatment
goals. Most patients use Home Health Rehabilitation services
after discharge from a hospital, a rehabilitation hospital, or
skilled nursing facility, but patients also come to us via a
direct referral from the primary care physician.

Pulmonary Services
Lee Memorial Home Health provides
comprehensive pulmonary rehabilitation care, in collaboration
with your physician, skilled nurses, physical therapists, and
occupational therapists. The goals of the program are to prevent
disease progression, relieve symptoms, improve exercise
tolerance, and prevent and treat complications. A skilled nurse
assesses and monitors the disease process, looks for ways to
reduce risk factors and works to manage things that make the
disease worse. Exercise programs set up by the therapist can
improve shortness of breath and fatigue. Patient education
provided by the professional improves self-care and improves the
ability to cope with illness and health status.
