Lee Memorial Home Health has changed its Cardiac Care Home program
to provide a higher level of customer service to continue its
quality care. The restructuring focuses on patient education
and employs a dedicated cardiac manager, Cathy Brady, to lead the
care for cardiac patients. “As a leader in home health care for
cardiac patients, we continually look for ways to improve what is
already an excellent program,” says Cindy Christman, Executive
Director of Lee Memorial Home Health.Cindy sought the advice of
cardiac physicians and nurses to provide guidance in key areas, such
as education, evidence-based clinical practice, outcomes and
technology. This group now makes up a Cardiac Advisory Board, which
meets regularly to discuss current trends and issues related to the
program.
This service is for patients who visit a physician for congestive
heart failure or other cardiac conditions, have had a recent
hospital stay, have frequent medication changes, have had multiple
chronic disease processes, have a medical necessity for
home health care services, are in need of medical education or have
a cardiac/heart disease diagnosis.
