Epic Works in the Community
Patients who visit their physician's office or are admitted to a Lee Memorial
Health System hospital experience Epic with each visit, but the electronic health
record system doesn't end at the door.
Rabia Khan, D.O., leads Lee Memorial Health System House Calls, a program that takes health care providers out of the traditional health care setting and into patients' homes. Dr. Khan and her team care for patients who are too frail to travel to a medical office or those who have just been released from the hospital and need extra attention in their home.
"I wouldn't do my job without Epic," Dr. Khan says. "Many of our patients aren't able to remember the medications they need or they can't recall some of their medical history. We are able to look up their medical condition and know immediately what medications they need and how best to treat them."
Because she spends her time in private homes, Dr. Khan relies on the high-tech system to help her when her patients may be living a low-tech lifestyle. "All of the information is right there," she says.
The system allows Dr. Khan to communicate with a patient's other physicians, creating a team approach to care. "We all know how a patient is progressing and what he or she needs to get better or manage an illness or chronic condition," she says. "It allows us to make a better health care plan."
