What to Bring to the Evaluation
In order to make the most
of your time, you should make sure to bring to your first
appointment:

Medical records
-
Recent laboratory results
-
List of current medications
-
Medicare card
-
Insurance card(s)
-
Picture ID
- Your calendar to schedule future appointments
- Any "Power of Attorney" or "Healthcare Surrogate" documentation
- Reading glasses or hearing aids (if you use them)
It is important for someone to come with you to these appointments.
If this is not possible, Please call the office so we can be aware prior to your arrival.
We are located at Lee Memorial Hospital, inside the Medical
Office Center on the 8th floor. When exiting the elevators on the 8th floor,
we are the first door on your left, Suite 814
