Bedside Delivery Program
As a service to our patients, prior to leaving the hospital, Lee Pharmacy can fill and deliver your discharge medication(s) to your bedside at no extra cost to you. If you prefer, you (or a family member or caregiver) may also pick up your medication(s) at Lee Pharmacy.
If you would like to participate in the Bedside Delivery Program, please fill out the Bedside Delivery Program consent form prior to discharge and fax it to:

- 239-985-3880 - Golisano Children's Hospital of Southwest Florida
Upon discharge, medications will be delivered to the patient's room. For additional information, please call Lee Pharmacy.
The Bedside Delivery Program is available from
9 a.m. to 7 p.m. and exclusively for Lee Memorial Health System patients being discharged from Lee Memorial Hospital, HealthPark Medical Center or Golisano Children's Hospital of Southwest Florida.
