HOSPITAL PATIENT SAFETY INDICATORS (PSI)

Lee Memorial Health System has been recognized over the last two years for its excellence in ensuring patient safety.

Hospitals who received this honor are ranked in the top tier in patient safety nationally, based upon a rating system using the Agency for Healthcare Research and Quality (AHRQ) Patient Safety Indicators (PSIs).

The AHRQ PSIs are a set of measures that were designed as a low cost screening tool for hospitals to use to identify potential problems and screen for adverse events and complications that patients may experience as a result of exposure to the health care system. These AHRQ PSIs provide a focus for further analysis and proven improvement to reduce errors throughout the System.

Note: In order to be consistent with current Florida hospital quality benchmarking criteria, the six Patient Safety Indicators presented below are the same six hospital AHRQ PSI measures as those publicly reported on the Florida Agency for Health Care Administration (AHCA) Compare Care consumer web site.

The number of patients reported as having a decubitus ulcer (a pressure ulcer or bed sore) after being in the hospital four days or longer, per 1,000 discharges with a length of stay greater than 4 days.

The number of patients reported with an infection, primarily due to intravenous lines or catheter, per 1,000 discharges.

The number of hip fractures on patients with an operating room procedure following surgery, per 1,000 surgery patients with an operating room procedure.

The number of patients reported with postoperative pulmonary embolism (PE) or deep vein thrombosis (DVT) following surgery, per 1,000 surgery patients with an operating room procedure.
