The Asthma Health Management Program is an education program for adult and pediatric patients as young as 2 years old. Referred by primary care physicians, pediatricians, pulmonologists and emergency department physicians, patients work with certified asthma educators to develop specifically designed action plans.
Based on the guidelines of the National Heart, Lung & Blood Institute, the Asthma Management Program is the only outpatient asthma education program in Southwest Florida.
By the end of the six-month program, patients should be able to self-manage their asthma, resulting in fewer absences from school or work, an improvement in quality of life, decrease in emergency department visits and hospitalizations and improved lung function.
