Scheduling Appointments
Schedule an initial appointment by

Referral requirements:
A referral from your physician is required to schedule an
appointment with a therapist. Once you have received the
referral please contact us at one of our following locations to
set up an initial appointment.

When you call to schedule
your appointment our office staff will collect information about
you including name, social security number, insurance
information, and
your reason for coming to therapy.
We ask that you arrive 30
minutes prior to your appointment time to complete the
paperwork.
Your initial visit will be
approximately 45 minutes. During this visit the therapist will
obtain a thorough history of your condition and perform an
evaluation. Treatment is initiated if time allows.
Your subsequent visits will
be scheduled following your initial evaluation and will be
approximately 40 minutes. These visits will be scheduled with
your therapist or therapist assistant.
Appropriate dress for
therapy includes clothes that will allow easy access to the
involved body part and that are comfortable for exercise.
