Prior to setting up your appointment our office
staff will collect insurance information from you and obtain
verification of insurance and authorization for payment. We
will also gather information from the insurance company on your
benefit limitations.
Please bring your insurance cards and your
drivers license to your initial appointment so that our office staff
can make a copy for your file.
Co-payments are expected at the time of service.
We gladly accept cash, check, and all major credit cards.
Lee Memorial Health System will submit your charges
to your insurance carrier for payment. Our office staff
will be more than happy to answer any questions regarding your
insurance and billing or direct you to the appropriate person.
