Center for Rehabilitation & Wellness performs customized and
individualized wheelchair evaluations to ensure proper fit and
function. We have set the standard of care for people
seeking solutions to mobility problems. Our services and
expertise are unmatched in Southwest Florida.
The individualized evaluation
includes:

A complete evaluation
performed by a licensed Physical Therapist in cooperation
with a wheelchair vendor to fully indentify and understand
the specific needs of the individual.

A functional mobility
evaluation (CMS compliant) designed to determine how the
individual performs mobility tasks associated with
mobility-related activities of daily living. This
assessment assists in defining the potential insurance
coverage for the devices considered.

Customized seating needs are
addressed

Recommendations to address
the specific needs of the patient to promote maximum
mobility both within and outside the home.
